Caremily Teams includes audit history for organizational coordination, but it should not be presented as a complete substitute for all records required by an employer or regulator.

Keep individual accounts

Shared logins make it harder to identify who performed an action. Give employees their own accounts and appropriate access. A history entry identifies the account involved, not necessarily every conversation or decision that led to the change. Discuss the context with the people involved rather than treating a timestamp as an explanation by itself.

Read changes alongside the current plan

A historical entry does not tell staff what to do now unless the current task and handover agree. Check the active client, the latest deadline and the responsible person. Correct the operational plan when needed. Do not promise immutable certification, automatic compliance assessment or clinical decision support merely because an audit screen exists.

Use the finding to improve the routine

If a change was recorded without handing over responsibility, agree a better confirmation step for next time. Keep the review respectful and limited to relevant information.
